New Client Inquiry

Thank you for your interest in working together! If you’d like to be contacted when a spot opens up, please take a moment to review the information below and then fill out the short questionnaire at the bottom of the page. If you are a provider looking to make a referral, please give this site link to the client requesting services to complete, as it is set up for potential clients and links directly to their client portal account. 

A few reminders before you fill out the form:​

  • Intuitive Wellness works with adults (18+), so I’m not currently able to see children or teens.

  • I provide individual counseling only - couples, marital, and family sessions aren’t available at this time. 

  • Appointments are scheduled only on Tuesday, Wednesday, and Thursday from 9am-4pm.

  • Before filling out the form, please review www.wvtherapist.com to learn more about my services, specialties, insurance, and rates.

  • In person sessions are available at my office in Fairmont, WV,

  • Telehealth sessions are available to clients located in WV, VA or PA at the time of their appointment.

Rate Information

  • Insurance - Depending on your insurance plan, your out of pocket expenses will vary.

    • You will be responsible for what your insurance plan requires as part of your contract with them (copay, coinsurance, deductible) and for any services/charges not covered by your insurance.

    • In Network Plans: Highmark/Blue Cross Blue Shield/Anthem, PEAK Health, and PEIA (UMR Plans Only).  Medicaid plans are not in network, even if they are provided to you by one of the above carriers.

  • Private Pay Rates

    • 50–55 minute individual session: $200.00

    • 90 minute extended session: $275

  • All Clients (Not billable to insurance)

    • No Show Fee - $150 billed directly to client

    • Late Cancellation (Less than 24 hours) - $150 billed directly to client
